A monoclonal peak with no prior data reflexes to immunofixation. Allow Gold tube to sit for 30 minutes to form clot. Spin for 10 minutes at 1100-1300 g in a swing bucket unit or 15 minutes in a fixed angle to separate cells from serum. Refrigerate spun tube. Remember to properly label tube with patient name, DOB, test and collect date. Minimum volume for ELP is 0.5 mL serum; for IFE is additional 0.5 mL serum.
Abnormal results will reflex to immunfixation upon pathologists interpretation. The Protein Elp Ser IFE Rflx test consists of a serum protein electrophoresis (SPE) test followed by an immunofixation procedure (IFE) when the pathologist reviewing the initial SPE believes there is a monoclonal band, or a possible monoclonal band, present in the SPE pattern. The results of this test include the pathologist's interpretation of the SPE and when indicated, also the IFE. Note: It is a Medicare requirement that this test must be ordered if the requesting physician would like to have a reflex IFE performed.
