Laboratory Order Requirements for Proper Ordering, Billing, Coding, and timely resulting.

IMPORTANT NOTICE: Beginning February 1st, 2025, Laboratory Requests that fail to provide the required information at the time of submission will result in rejection of the laboratory testing.

ACTION: When submitting laboratory orders and testing, each of the following requirements is needed for each submission for proper ordering, billing, coding, and timely results.

REQUIREMENTS: Failure to include this information impacts patient care and may lead to delays, rejection, specimen recollection, and insurance coverage denials, resulting in unexpected out-of-pocket expenses for patients.
 
  • Patient Information: All patient specimens must be labeled with the patient’s full legal name and date of birth as well as the date and time of specimen collection, and collectors initials. Patient Information on the Paperwork must match the specimens submitted.
  • Payment Type: Specify if the payment will be billed to Patient Insurance, Patient Self-Pay, or Client Billed. If billing to insurance, complete insurance information is required for EVERY submission.

If the patient is under 18 and billed to Patient Insurance: In addition to the required insurance information, please include the primary subscribers full legal name and date of birth.
 
  • Provider Information: All requests for laboratory services must be authorized and signed by the patient’s ordering physician. This ensures that the request is legitimate and has been approved by the responsible healthcare provider.

Signature Guidelines: Orders/Requisitions must include the legible signature of the physician or non-physician practitioner (NPP) responsible for and providing care to the patient. The signature must be legible and signify knowledge, approval, acceptance, and/or obligation. Per CMS guideline Copies/Stamps of the physician’s signature cannot be accepted. Electronic signatures will be accepted if it is explicitly stated on the submitting order.
 
  • Diagnosis: The appropriate ICD-10-CM codes must be provided with each requisition. These codes should reflect the specific clinical condition that warranted the laboratory testing at the time of service and be specific to the patient's condition. An Advanced Beneficiary Notice (ABN) is required when testing is ordered that may not meet a covered medical necessity.
 
  • Testing Requests: Clearly state the requested test, using standard testing names without abbreviations. When available, provide Carle Health test codes. For testing not in the test directory or esoteric testing, provide the specific ARUP test code of the test being requested.
 
  • Specimen Collection Details: The date and time of collection must be clearly indicated on the order requisition(s)/script. This will be used to indicate the fate of service as well as aid in specimen stability.
Please Note: The collection date and time on the order MUST match the date and time on the specimens submitted with the orders.

TEMPORARY SPECIMEN RETENTION POLICY CHANGE FOR METHODIST CAMPUS                                       EFFECTIVE 4/29/24

Due to ongoing Laboratory construction, there will be a temporary adjustment in our specimen retention policy.  The retention period for Serum and Plasma specimens will be reduced to 5 days start on April 29, 2024.  This change will remain in effect until further notice and only impact Methodist Hospital and Carle Health Methodist Reference Lab Clients.

Please note that some testing may indicate stability for up to 7 days within the Laboratory Test Directory.  This
does not guarantee specimen availability to retrieval and/or testing.  Serum and Plasma specimens in laboratory storage will only be accessible for retrieval within the initial 5 day period.

We apologize for any inconvenience this temporary adjustment may cause and appreciate your understanding during this transitional phase.  Should you have any questions or require further assistance, please feel free to reach out to our Laboratory Customer Service Team.
