Ordering Recommendations

Urine culture is performed to aid in the diagnosis or management of Urinary Tract Infections (UTI) and is not intended to detect colonizing flora, including Streptococcus agalactiae (Group B Streptococcus). For questions/ordering related to colonization with Streptococcus agalactiae (GBS) during pregnancy, see test codes GBS or GBSS which utilizes rectal/vaginal specimens.

Patient Preparation

MIDSTREAM COLLECTION:
Wash hands thoroughly with soap and water, rinse and dry them on a disposable paper towel.
Female: Open the towelette and cleanse the urinary opening and surrounding area.  Discard the towelette in the waste basket.  Void 20-25 mL into the toliet and catch a portion of the rest of the urine in the container without stopping the stream.  Place the lid securely on the specimen cup.  Transfer specimen to boric acid preservative tube (gray top).
Male:  Open the towelette and cleanse urinary opening.  If not circumcised, hold foreskin back prior to cleaning.  Discard the towelette in the waste basket. Void 20-25 mL into the toliet and catch a portion of the rest of the urine in the container without stopping the stream. Place the lid securely on the specimen cup.  Transfer specimen to boric acid preservative tube (gray top).

Collect

Midstream urine, catheter, bladder tap, kidney or other special collection method. Source of specimen is required.

Storage/Transport Temperature

Urine for aerobic culture should be submitted to the lab in a gray top urine preservative (boric acid) transport tube at 4-25°C.

Unacceptable Conditions

Nonsterile or leaking containers, delayed transport to the lab, urine from catheter bag, Foley catheter tips.

Blue top needle collection containers are not acceptable in the lab for processing.
 

Remarks

Gram stain is performed by request only. Results indicate colony count plus identification of significant isolates. Anaerobic culture not routinely performed except for suprapubic tap submitted in sterile-capped syringe (needle removed) are acceptable for anaerobe culture.  Specify if Neisseria gonorrhoeae or Salmonella typhi are being sought.
