Collect

For facilities sending blood culture specimens to Memorial Central, collect BD Bactec plus bottles with the long necks. Refer to the MHC, GVH, PPRH, FSED, and Select Outpatient Labs Blood Culture Collection Tool HERE for bottle descriptions and minimum volumes.

Remarks
For UCHealth Colorado Springs/South Region current inpatients and ED patients, a molecular (PCR) organism ID panel may be performed and resulted approximately 2 hours after the positive blood culture gram stain is initially reported in Epic. This panel will be performed once per patient, every 7 days, and will not be repeated unless subsequent positive blood culture gram stain shows a different organism.  In most cases, it will not be performed on gram positive rods (call Microbiology for exceptions).
 
The molecular (PCR) organism ID panel will detect the following organisms and associated resistance markers in parentheses:


Gram-Positive Bacteria
  • Enterococcus faecalis
  • Enterococcus faecium
  • Listeria monocytogenes
  • Staphylococcus spp.
    • Staphylococcus aureus
    • Staphylococcus epidermidis
    • Staphylococcus lugdunensis
  • Streptococcus spp.
    • Streptococcus agalactiae (Group B)
    • Streptococcus pneumoniae
    • Streptococcus pyogenes (Group A)

Gram-Negative Bacteria
  • Acinetobacter calcoaceticus-baumannii complex
  • Bacteroides fragilis
  • Haemophilus influenzae
  • Neisseria meningitidis (encapsulated)
  • Pseudomonas aeruginosa
  • Stenotrophomonas maltophilia
  • Enterobacterales

The BCID2 Panel also contains assays for the detection of genetic determinants of resistance to aid in the identification of potentially antimicrobial resistant organisms in positive blood culture samples. The panel detects the following:

Antimicrobial resistance genes
  • mecA/C (for Staph. epidermidis and Staph. lugdunensis)
  • mecA/C and MREJ (for MRSA)
  • vanA/B (for Enterococcus faecalis and Enterococcus faecium)
  • CTX-M (for gram-negative rod targets)
  • IMP (for gram-negative rod targets)
  • KPC (for gram-negative rod targets)
  • mcr-1 (colistin resistance for select gram negative rods)
  • NDM (for gram-negative rod targets)
  • VIM (for gram-negative rod targets)
  • OXA-48-like (for gram-negative rod targets, excluding Pseudomonas and Acinetobacter)
Conventional culture will follow the molecular (PCR) organism ID, and susceptibility will be performed on appropriate organisms.
